
Talking Nutrition In Chronic Disease

This one day interactive workshop will equip GPs, practice nurses and other healthcare professionals with scientific facts and brief intervention concepts around diabetes, cardiovascular disease and more. Building on the use of the conversation as a tool for change, this course will examine how nutrition can impact QOF outcomes and multimorbidity challenges. Throughout the day there will be a focus on over- and under-nutrition. As well as a practical look at how to effectively convey nutritional messaging and empower patients to make use of local social prescribing options.
LEARN IN DEPTH
- Gain evidence-based insight into the impact that diet can have on common multimorbidity disease clusters
- Look in detail at cardiovascular disease, type II diabetes and other metabolic disorders to understand shared risk factors and nutrition impacts
- Take away a workbook including case studies, fact sheets and discussion topics for ongoing learning

- Understand the basics of Motivational Interviewing
- Translate theory into practice through role play and small group discussion
- Establish a practical plan for addressing chronic disease and nutrition throughout your workplace
